Have to admit googling... and can't believe it turned out to by my team and I didn't know... but was it Roy Carroll in 2005?

Didn't Man U lose a penalty shoot-out to Arsenal that season after a goalless 120 minutes?
Carroll did concede in the penalty shoot-out!

I’ll put you all out of you misery.
The goalkeeper in question was Ray Wood of Man Utd in the 1957 final against Aston Villa. He was knocked unconscious and broke his jaw following a challenge by Peter McParland forcing him to leave the field. Jackie Blanchflower took over conceding two goals, Wood returned to play on the wing, that was until United pulled a goal back wherein he returned between the posts for the last ten minutes as United searched for an equaliser.
